We still experience this bug on lucid today. Bootchart shows that the
boot stalls on mountall.
The system boots in some cases, so it is possible to compare OK and KO
cases. Attached are:
* The two bootcharts (OK and KO) ;
* The two mountall --debug logs (OK and KO);
* Our /etc/fstab
The bootchar
Sorry I could only add one attachment, so here is a tarball with all the
attachments listed in the previous comment.

Waiting for /some/partition
Upon debugging further, it seems mountall is waiting for
/dev/HEBEX/WOO_BASE, and rightly so. After skipping with 'S' during
boot, here is what we see:
r...@malorent:~# lvscan
ACTIVE'/dev/HEBEX/WOO_BASE' [1.00 GiB] inherit
ACTIVE'/dev/HEBEX/WOO_LOG' [1.00 GiB] inherit
This could be linked to bug #527666 where mountall makes boot stall with
some LVM devices missing.

johe: can you please give the entire output of
* ls -l /dev/mapper
* lvdisplay | awk '/LV Name/ { print $3 }' | xargs ls -l
In our case, this is what we get:
r...@malorent:~# ls -l /dev/mapper
total 0
brw-rw 1 root disk 251, 4 Apr 23 14:57 HEBEX-VAR_LOG
brw-rw 1 root disk 251, 2 Apr
Add to this a search in /var/log/udev:
r...@malorent:~# grep mapper /var/log/udev
KERNEL[1272027429.282338] add /devices/virtual/misc/device-mapper (misc)
DEVPATH=/devices/virtual/misc/device-mapper
DEVNAME=mapper/control
UDEV [1272027429.914560] add /devices/virtual/misc/device-mapper
This can be traced in the udev links, too:
r...@malorent:~# ls /dev/.udev/links/ | grep HEBEX
HEBEX\x2fVAR_LOG
HEBEX\x2fWOO
HEBEX\x2fWOO_LOG
HEBEX\x2fWOO_PROG
disk\x2fby-id\x2fdm-name-HEBEX-VAR_LOG
disk\x2fby-id\x2fdm-name-HEBEX-WOO
disk\x2fby-id\x2fdm-name-HEBEX-WOO_LOG
disk\x2fby-id\x2fdm-name-H
Update: the /dev nodes seem to only be missing when two LVM partitions
fail.

scott: just tried 2.14~ppa1 but it doesn't fix the issue (which we
believe to be also a dupe of bug #527666 and bug #540155). We keep
getting random LVM mount issues at boot.

>From what I see, the ppa build fixes the buffer to listen to udev.
However, the udev logs don't even list the missing devices
(/var/log/udev) which makes us think that udev itself doesn't know about
the device.
lvscan, however, sees it fine.
