I have now reproduced this on Unity 5.12.0 with Ubuntu 12.04 LTS. I'm
attaching a partial screenshot showing the affected launcher bar.

I left my computer on overnight. I usually find it is caused by leaving
the computer idle for some time. The screen saver or power save for the
monitor turns on and then when I unlock it is in that state.

I also recently expreienced this issue. I haven't restarted my system
in about a month, then had some problems with the sidebar not getting
out of the way of windows. Then it wouldn't pop out and now I am having
this issue where all icons are black but when I hover over the window I
can read the
output of glxinfo

I have a couple of other observations:

This issue, to me, is the same issue as
https://bugs.launchpad.net/ubuntu/+source/unity/+bug/762478. However a
poster on their thinks this is only related to the nvidia graphics
driver. I tend to disagree, nonelethess I am filing a new bug for it.
The behavior is the same.

I find that my launcher also doesn't automatically hide at all times.
It _usually_ hides when I move an application over the same window
space. This doesn't always occur. Same goes for when I maximize a
window.

I've taken a screenshot. Notice that when I hover over the location
where the icon should be, its title shows up.

killing the xorg process causes a restart and gets things going again
without rebooting, but all your open gnome-based applications are closed
also. This is a very, very, very, very annoying bug.
