It seems that regular Blender (version 2.45) provided by the package
manager does not have an icon. Blender 2.46 downloaded from blender.org
site on the other hand has the icon in the window. Same is true for a
recent SVN version.
I am not sure what is causing the issue. Perhaps there was some fix

general Blender does not use a lot of confirmations. This is might be
something that will be looked upon during the development cycle of the
next major version, 2.50.

A fix for this "bug" is on current SVN version of Blender. Also current
version of Blender does run in windowed mode by default now.

Does glxgears run alright for you? In case you are using dynamically
linked build proper OpenGL support is required. It might be possible
that your graphics card driver is not up to date. Sometimes this happens
because of a kernel update.
