It seems to complain about pam_shells.
And indeed, after commenting this out, the problem goes away, even with
both anonymous_enable and local_enable set to YES.
The problem becomes different when starting vsftpd from the command line
instead of using "service vsftpd start":

Found out what happened.
1. The Kubuntu installer actually does correctly move away initctl (and
other similar programs) during install of main system (using the /lib
/chroot-setup.txt script)
2. However, in my case, install failed due to other issues. This made
the "in-target" wrapper fail, and
Chuck, the comment you replied to didn't really confirm that the problem
was solved. More to the contrary, the problem still exists, and even
was so bad that Craig gave up on it and decided to just wait for the
next version...
... which eventually turned out to be as broken as 9.04 because nobody
I can no longer reproduce it on current versions of Kubuntu 10.04
But I suspect what happened was a combination between bug 574808 (RAID
issue causing the actual failure to boot) and 574814 (display issue
hiding any indication of what really went wrong).

Comparing with an old (9.04) kubuntu, I noticed that /usr/share
/initramfs-tools/scripts/functions contained a definition for
add_mountroot_fail_hook, whereas the same file for 10.04 no longer
