When is it over 0 dB? I set the sound volume only through the standard
Ubuntu indicator and the built-in application sound indicator.

Nope.
Actually the problem is GNOME related. If you adjust the keyboard layout
to be change with "Ctrl+Shift" (and not as is by default with
"Alt+Shift"), these keys are blocked by GNOME and the keyboard layout
changer.

It is as if two layouts are active at the same time and the output is 4 letters

I've made a new clean installation and the problem occurs again.
I've found a work-around:
"Separate layout for each window" and "New windows use active window's layout"
should be turned off.
