I believe it happens when stacking gets broken in compiz (bug 845719) but have
no way to confirm it for sure. One other thing is that when it happens alt-tab
doesn't display its switcher either but it does switch the windows. I can
access launchbar by pressing super key or by moving the cursor t

I don't know how to reproduce it. It happened with a fresh boot. I
opened Google Chrome (which was not maximized yet), maximized it, then
noticed I couldn't access the Unity menu. The Super key worked fine. I
