This happens as well with mplayer (just try mplayer some_video_file in
terminal). Now, mplayer does not have a desktop file (bug 857243) but I
would expect it to show in Unity with a questionmark icon (as it stands
now, it does not seem possile to alt-tab to mplayer in unity).

It happens to me too on a freshly installed Oneiric (AMD64).
The problem manifests itself quite consistently when I open terminal,
but yesterday it happened a couple of times with Chrome too. I'm sure
sometimes it doesn't do it because I was able to select "Keep in
launcher" for the terminal. When
Tested on Oneiric. Shows up fine for me. If you find a way to replicate
this with current Oneiric, please reopen this bug.
