This is how the button behaves upstream (it's only visible if you hide
the menubar), so we aren't going to change that.
And a lot of functionality that is present in the navigation bar has
already been removed from the menus too in Firefox 4 (Stop, Reload,
Back, Forward, Open Location, Close Tab/W
Hmm, why not leave it up to the users if they want that button there or
not. There is a lot of functionality in the navigation toolbar that can
also be accessed from the menu. And this one is quite confusing, you add
that icon in the "Customize" view but it disappears later.

That's by design. It only shows if you hide the menubar (it makes no
sense to show the button when there is a menubar too). However, if
you're in Unity then you can't hide the menubar
