Ok, so this is nautilus breaking the .desktop file specification as can be
found http://standards.freedesktop.org/desktop-entry-spec/latest/ar01s02.html
"Multiple keys in the same group may not have the same name."

This bug does not happened when icon is changed during creating .desktop
file. This happen when icon is changed by properties in nautilus (right
click -> properties). So that this is not probably problem of unity. In
the .desktop file are two different address of icon. On the desktop is
shown icon
Can you please post the contents of that .desktop file?
