After two years, this bug is not fixed. Also i've replaced my computer
with a new notebook. Now, i am not having this problem anymore.

This bug is fixed by latest update this week. But when I checked; grub
version is the same with this bug report. I think this bug was related
to another component.

I don't know if it helps but, recently it does not boot after keyboard
ctrl+alt+del restart. But after hard restart from restart button on the
computer case fixes the problem until next cold boot. Also there is no
problem with restart functionality of the ubuntu when it is used from
shutdown menu.
