The crash is unreproducible in either Ubuntu 10.10 or Ubuntu 11.04

Now I use libreoffice, but this bug is always there.
Here is the file. Double clic to open form, then crash when cliquing on
the form

I could not reproduce this problem in Ubuntu 11.04, LibreOffice Base.
Could you please attach a compressed base file that demonstrates this
problem?
