I haven't had this problem since, so I guess not. I have just tried to
reproduce it by opening the same document, and it doesn't happen, all
the icons display properly.

I've just waded through the attachments that were posted automatically
when I reported this bug, and thought I could see something about glib
which looked (to my untrained eyes) serious - does this affect it?
