Did you make sure that the email with only an attachment does indeed not
contain a MIME container and has the email's body only as the image? The
headers of such an email, as can be seen using "View message source"
should look something like this:

working fine here, which kind of attachment is that one? I've attached
an jpg picture and after forwarding it to another email address without
putting text on the body, the attachment gets sent anyways, not
confirming here, thanks.
