I have also seen this kernel oops. For me it is repeatble if I try
"iwlist scan" when then card is not yet enabled through the network
manager. If the card has been enabled then everything works fine.

I can replicate this with an identical stack trace. The madwifi driver stops
working after this, which is sort of disappointing.
