The nvidiavdpauppa11 build in the nvidia-vdpau ppa fixes the fontconfig
patch not being applied, and always had the xscreensaver patch.

Just updated it, and bug is still there. I've also compiled from source
and bug is there. Added the disable-xscreens... to command line and
config file, and bug is there. It was NOT there in Ubuntu 9.04.
INCREDIBLE!!! No developper paying attention to this? Then why continue
releasing new versions?
At the moment I use the nvidia-vdpau ppa, and that repo has it's own
mplayer-package (2:1.0~rc3+svn20090620~karmic~ppa4), which is to the
best of my knowledge unpatched regarding screensaver and font dirs (bug
#448603).
The 2:1.0~rc3+svn20090426-1ubuntu10 version works as expected for me.

Bug #468721 was filed according to the request of siretard. On November
01 the bug is still there.
