I was able to very easily reproduce this one. Get this package:

Use it to parallel SSH to multiple machines (at least 10) and execute a
simple command (like ls). Do it 2 or 3 times, and your ssh-agent will
freeze.

I forgot to mention, not just the ssh is what hangs, apache and other
services hangs too. And after a while (say 5 minutes) it all becomes
responsive again. Until I try a couple of commands / navigate some dirs
/ edit some file that it hangs again.

This is happening to me since ubuntu 9.04, when I connect through ssh (from mac
and linux and windows clients it's the same) it hangs after a while, generally
after going through the command history, but it also happens when navigating
directories or trying to edit files.
When I had ubuntu 9.04

Looks like the same here: ssh-agent is frozen.
The only way to get an outgoing ssh connection was to:
unset SSH_AGENT_PID
unset SSH_AUTH_SOCK
ssh-agent would not even die on a kill -9
