The Qt frontend to checkbox debuted in Ubuntu 12.04, and it clearly
labels the Email field along with a short set of instructions. Marking

This. It took me three tries to successfully submit, because I didn't
realise there was even an email entry field. It looks like just a
spacing element because it is the whole width of the window and
unlabelled.
