I performed hibernation for several times without a problem. I just hope

When I said attach the output of the command, I meant attach it as a
file...
Anyway, I had the same problem as you months ago and I fixed it by downloading
and installing uswsup, so type
sudo apt-get install uswsusp
After that, you have to edit the /etc/uswsusp.conf file with th
swap: 3 gb, ram 512 mb
lspci -vvxxx puts next:
