Ubuntu just downloaded a Poppler update today (0.8.7). I printed the
offending file on the offending printer and everything is correct.

Yes, printing to a postscript or PDF file works properly. Only observe
the problem when printing to a Brother MFC 9800 and Samsung CLP600
colour laser printer.

I've tried Acroread (Adobe), XPDF and PDFedit. All three work fine
(well, PDFedit I can export a PNG that looks fine).
Here is a scan of what I see (top is the Acroread print, bottom is
Evince). I should add that I'm using Evince through the Ubuntu Document
Viewer 2.24.1, which claims to use
Alex, the pages (2, 5, 6) print fine on my machine...
Could this be your printer?
Try using another pdf viewer to see if it's the printer driver or the pdf
viewer...
