it's the same for me, I've set my print my default print size as A4, but with
firefox still send in letter format. OpenOffice and Ms outlook and Word via
CrossOver work well.

I've noticed that Firefox doesn't print exactly Letter when you select
A4. It seems in my case that it simply prints the contents too close to
the top edge of the sheet, so a large white margin at the bottom
appears, and so it looks like a Letter output.

I managed to overcome this by modifying the following settings in about:config
print.postscript.paper_size : A4
print.print_paper_height : 297,00
