I had something similar happen to me. I was using a custom .gtkrc-2.0
file in my /home that I had forgotten about. When I cleared it out, the
problem went away.
Something to try/consider.

I upgraded this installation from an untouched Gutsy installation at the
beginning of the cycle, and it has all updates installed.
$ apt-cache policy human-theme
human-theme:
Installed: 0.16
Candidate: 0.16
Version table:
*** 0.16 0
500 http://archive.ubuntu.com hardy/main Packages
Ca you please provide more information about your installation (update
or fresh install). If you haven't updated in a while I suggest doing so
as we included these changes a couple of weeks ago and apparently nobody
else can reproduce this bug.

the index.theme file points to Human metacity theme so I am not sure why
this is happening. When I create a new user on my system the theme shows
up as "Human" as it should.
