Had the same problem yesterday (error inserting ath_pci) but today I
followed instructions from
http://madwifi.org/wiki/UserDocs/Distro/Ubuntu: add ath_hal to
/etc/default/linux-restricted-modules-common and rmmod all ath_hal,
ath_pci, wlan modules. Then I cleaned and rebuilt madwifi from svn.

One solution here : rebuiling Madwifi didn't help but de-checking - re-
checking it in restricted system/hardware drivers and restarting fixed
it

Update as well : it was working for me on a fresh install, using kernel
2.6.24-16-generic, but upgrading kernel to 2.6.24-17 broke it, so it
would seem any upgrading will cause it to break

I'd like to post an update. I upgrade from 7.10 to 8.04. Wireless worked
from kernel 2.6.24-12 up to -15, but not -16. I decided to do a clean
install when the hardy final release came out. The wireless on a clean
install, using kernel 2.6.24-16-generic, works well. So, it would seem
that this bug
