As you can see, this has been added as a feature in Okular (which is the
kpdf for kde4) http://bugs.kde.org/show_bug.cgi?id=152708
The first page can be centred and then the 2nd is on the left hand side,
the 3rd on the right hand side.
I believe the report can be closed.

But then it's not the SAME document. (And requires a tool that would
insert a page of the right type at the beginning of a PDF.)
(and this makes all kinds of assumptions that the user has sufficient
permissions to alter the PDF [perhaps it's signed; perhaps it's on a
read-only filesystem; etc., et

However, I believe this is not correct. If you use View->Two Pages, I
believe your feature request is already implemented.
Therefore, I would like to close this report if you agree.
