In my last comment, I meant to say that I downgraded to 3.5.8-0ubuntu1 :) I just compiled kdeutils 3.5.9 from source, and this problem seems to be fixed, though one of the patches needs to be tweaked to get it compile. I'm attaching my modified patch file for that in case someone finds it useful.

I downgraded kmilo to 1.5.8-0ubuntu1 from here... http://archive.ubuntu.com/ubuntu/pool/main/k/kdeutils/kmilo_3.5.8-0ubuntu1_i386.deb and the problem went away, so it must be a new bug in kmilo_3.5.8-1ubuntu7. I'm marking this as confirmed and putting the package as "kdeutils" as that's what src

Ah yes, now that you say it I have something similar here: using the hot keys adjust both volume and something named "3d control - depth", and when the latter reaches 100% the volume doesn't increase. (besides, I have no idea what this 3d control - depth does...) This is hardy with latest updates
