Update: I upgraded to 4.2.0-38-generic; and both the dash and launcher
showed up; but there was a problem with the resolution and missing mouse
pointer, which were resolved with rebuilding the Virtual Box Guest
Additions.

The display issue reported in this bug is resolved.
I did notice a shared clipboard issue (but that may be related to me
needing to re-build the vbox guest additions on the new kernel).

specific source package though, rather it is just filed against Ubuntu
in general. It is important that bug reports be filed about source

Forgot to mention; as part of the different steps to resolve the
problem, I did upgrade to 16.04 on a snapshot of the same VM; and the
same problem re-occurred after a kernel upgrade (don't remember the
kernel versions there).
