I had the exact same problem. I uninstalled evms and now my partitions
are mounted without problems. I don't know if that is the correct
solution, but it worked for me. YMMV.

Even the swap cannot be mounted with UUID...
In fact, only the root partition / is correctly mounted using UUID...
Changing the fstab back to /dev/mapper/sdxy solves this problem...

Of course, it is not a problem with /media/win_d... the error occur whatever
the destination directory.
And the problem also occurs with sda2 which is ntfs
