ran in to this bug for trusty, for now using Gabriel Devenyi's PPA and
it's working fine.
[seems like multipath v0.5.0 do not like wwid: "*" in blacklist
section but that's for some other day :) ]

sg3-utils_1.40-0ubuntu1
it resolved the problem.
so the problem is in the old multipath-tools package in trusty.
