the updates through 3.19.0-33 seem to have resolved this situation. grub
runs correctly after dist-upgrade & system reboots normally.

You shouldn't be getting that bios compatibility warning unless you have
a disk with partitions on it, none of which are an ESP. Do you have a
second disk configured in the vm that you aren't installing to? Are you
sure you clicked continue at that warning, because that should make it
pick grub-e
And here's the screen with the warning message. I pressed "continue"
which the text implies to me will continue with UEFI install.

I agree the partman log seems to show no EFI partition yet the parted
list shows one their and it is mounted.
I've now done an Ubuntu build with screenshots for you.

I did get the popup about a non-UEFI system and thought I had chosen the
correct option for UEFI. Will include when I create screen shots for

Right. This would most likely be because there is a popup which asks
whether to ignore the fact that the system is booted in EFI, and it
interprets the answer as "continue without EFI". In this case, things
revert to using mostly non-EFI code paths (so, no automatically made
ESP).

Yesterday I ran 6 installs in VirtualBox. If I used automatic
partitioning I got grub-efi. If I used manual partitioning I got grub-
pc. I was rather surprised when I realised my real machine had booted
with grub-pc on a GPT disk without a bios_grub partition but it did.

The original install where the problem was experienced was done with
only one disk in place, with root in sda4 and EFI in sda1, swap sda2.
These were set up with manual partitioning. No RAID.
After I had removed grub-pc and installed grub-efi I added the extra
disk and raided sda3/sdb3 and moved
