I have reinstalled all of them, now every icon is there.
Error in the unity8.log file:

Sorry about that, no one else ever reported such an issue... Do you
still have some of the items with missing icons or have you reinstalled
all of them? If you could please look in ~/.cache/upstart/unity8.log or
unity8-dash.log (and the .*.gz ones too, they're the older ones) for
relevant image loa
Every app which has no icon in the app scope has no icon in the launcher when
launched. In this picture it was the Telegram app, but every other one has the
same behaviour.
Unpining and repining it again has no effect.
The solution was to deinstall the app and reinstall it. Then the icon is back.
The issue in Apps is likely bug #1430828, it's going to be resolved in
the first vivid-based OTA.
The launcher issue, however, is new to me, can you identify what app is
it? If you long-press, unpin it and pin it again, is the icon back?
