Christopher, I've reproduced it with current Vivid beta and kernels:
v3.7.10-raring (mainline kernel)
3.19.0-12-generic
3.19.0-13-generic
v4.0-rc7-vivid (mainline kernel)
I only recently tried the BIOS option, after I upgraded to Vivid beta,
so I've also tried these LTS Live CDs now:

Christopher, I'm pretty sure it did but I haven't used the BIOS option
before. I went looking for a VT-d option in the BIOS and figured that
AHCI is better than IDE. I was wrong. :-)
"IDE Mode" is the default.

I've tested the v4.0-rc7-vivid kernel now and the bug is there too.
A "PCIE-to-SATAII/IDE RAID Controller BIOSv1.06.65" Option ROM kicks in
in "AHCI+IDE Mode".
Noted differences:
Working IDE Mode:
pata_jmicron :04:00.1: enabling device ( -> 0001)
ahci :04:00.0: AHCI 0001. 32 slot
Patrik Lundquist, just to clarify, if you uninstall the nvidia driver,
can you then test the 4.0-rc7 kernel?
