I've had the exact same problem, and only realized that the "Show sound
volume in the menu bar" option in System Settings > Sound was somehow
unchecked after Skype was installed. I got the third error after trying
to reinstall pulseaudio to fix the problem, but it was gone after
accountsservice-ubu
