Great, that solved it for me.
I'm still wondering why it was like it was but overall I'm happy to be rid of
the unnecessary button.

Found a lighter solution to this bug here :

1- go to "about:config" and set "ui.use_unity_menubar" to false
2- right click on the menu bar and tick "Menus bar"
3- set back "ui.use_unity_menubar" to true in about:config

/en-US/kb/reset-firefox-easily-fix-most-problems) fixed this issue for
me.

Small addition is that the menu button can not be removed.
When in Private Mode I also get two icons, normal Private Mode icon and Private
Mode menu button.
