on Karmic (upgraded long ago from Jaunty xubuntu)
I cannot add anything with alacarte, either as a normal user or as root
(sudo alacarte or gksu alacarte)
dpkg -l alacarte
ii alacarte 0.12.4-0ubuntu2
~/.config/menus and ~/.local/share/applications are belongi
I'm closing this bug report for now, would be nice if someone may test
this with our development release of Ubuntu the Gutsy Gibbon you may
grab a CD Image from here: http://www.ubuntu.com/testing/ feel free to
re open it if you may test it with that release.

So, check if ownersip of files is correct.

I cannot reproduce, can you attach the contents of ~/.config/menus and
~/.local/share/applications?

Interesting that I am able to add items to the root menu, just not under
system tools...
