I haven't had this problem since, so I guess not. I have just tried to
reproduce it by opening the same document, and it doesn't happen, all
the icons display properly.

I do have a NVIDIA graphics card, that probably has a lot to do with it,
in fact it might not even be related to Ubuntu at all in that case...

I upgraded from Karmic to Lucid Alpha 3 earlier on today and have just
properly rebooted since the upgrade and noticed that the boot splash
appears when it shuts down but not when it boots up (the aubergine dots
thing)!

different documents (not that the documents should make any difference)
and the icons still didn't show up. I'm using Ubuntu 9.10 (karmic).
Hopefully the Report a Problem feature will have gathered lots of useful
information.
This bug affects me too, sometimes, and it's very annoying! It seems to
be working for me over recent days but before it wouldn't. Thanks!
