I just noticed a problem with my earlier post. While I noticed that the
ath5k module was loaded, it was loaded alongside the ath_pci module.
The working internet connection was actually using the ath_pci module.
After removing the linux-restricted-modules packages, NetworkManager
tries to connect

The steps provided by Alberto in comments 1 and 3 appear to have fixed my
problems with the nvidia driver.
glxgears is now happily reporting:
25952 frames in 5.0 seconds = 5190.263 FPS
I'm not entirely sure what the steps did, so I can't really describe
what the actual problem was.

I can verify that fail2ban still doesn't start properly.
/var/run/fail2ban is not being created on startup so fail2ban will not

I noticed the problem occurring again after the mythtv theme was changed
from the black back to the blue theme. mythubntu is still running the
black theme by default. when the problem occurs, I switch to a VC and
run 'top', but it doesn't show any process consuming large amounts of
CPU or RAM.

I tested both rt2x00-test-v3 and -v4 on

rt61 pci card connecting to a hidden WPA2-AES network.

I can verify that the r8169 driver on 8.04 only sometimes connects to
dhcp. Restarting the computer fixes the problem. This doesn't always
happen though.

I can verify that the rt61pci driver included with the latest hardy
kernel doesn't work properly with my D-Link DWL-G510 rev. C2 PCI card.
The driver detects the card, but will not connect to a hidden
WPA2-Personal (AES-CCMP) network.
