I don't think the applications should be removed from the menu because
it isn't necessary for it to be launched with a chm file, I mean, if you
try to open it from the menu it runs.

Instead of using nvidia-settings's one, I used a icon from Envy package
from Alberto Milone, which is released under GPL; I hope this is ok. I
updated debian/copyright as well, but I don't know if it is ok.

I got that error too, after installing compiz-fusion. I resolved it
editing desktop/gnome/applications/window_manager/default in gconf-
editor back to metacity.
