pulseaudio crashes / doesn't start

Pulseaudio seems to crash constantly. The icon in the notification area
always shows no sound. I am unable to adjust the sound with the sound
indicator nor with the Macbooks sound keys.
My /var/log/syslog is filled with the following messages:
Feb 19 16:07:13 laptop616 pulse
I have the same error with external USB 3.0 disk. It happens both on Ubuntu
11.10 and 12.04, and both when using USB 3.0 and USB 2.0 ports.
Error detaching: helper exited with exit code 1: Detaching device /dev/sdb
USB device: /sys/devices/pci:00/:00:1d.7/usb1/1-1)
SYNCHRONIZE CACHE: FAILE
My USB 3.0 disk only unmounts partitions, it doesn't power off.
