This happened in a VM, when I tried to launch Cadence Virtuoso from the
HGFS partition. I think I'm ready to blame VMware for this, but I
figured I should submit it up anyway just in case VMware just
exacerbated a real bug in the kernel.

On my laptop, which has intermittent problems with USB being plugged in
and unplugged, pulseaudio locks up if the USB port is acting up and has
connected and disconnected the device many times in a row. It seems
potentially related to #300290,
Happened for me when I tried launching apport in --file-bug --pid mode
with both apport-cli and apport-gtk. The workaround proposed by goto
(wait a moment, then refresh) seems to have worked.
