I believe the issue was introduced between Kernel version 3.17.8 (I
can't reproduce the issue) and 3.18 (I can reproduce the issue), I'll
start digging through the commits and report back on what I find.

I've tested this the new systemd and upstart boot options and I've now
been able to reproduce with both. I can't consistently reproduce the
issue until it occurs. Once it occurs, it's completely consistent until
reboot. I didn't experience this issue in 14.10 or 14.04. Let
I've experienced this issue with a Galaxy S4 running 4.1 and now a
Galaxy Note 4 running 4.4.4.
