Confirmed that this is because of a hardware error with this particular
adapter. I tired another identical adapter and it works fine. Tested
the bad adapter on 32bit Ubuntu and same issue occurred. So this is not
a bug but an error thrown due to a damaged adapter.

This particular adapter worked with fresh install of Kubuntu Hardy x86_64
Now using kernel 2.6.24-18.
