I noticed that if I comment out the loading of the `module-card-restore`
in `/etc/pulse/default.pa` then this problem does not occur. The echo
cancelled devices are present on the first run. Of course, I do like
having my cards restored!

I have no resolution until I manually add a `nameserver 8.8.8.8` to
`/run/resolvconf/resolv.conf`.
And if `resolvconf` is not installed, I don't know how to add a
nameserver. So I keep that installed.
I'm using Network Manager and it should take care of adding nameservers.

Totem, seems to work better, though. It fails to play at all. Which is
better than playing the video and playing very loud noise with it.
