I got it! At some time I must have changed the session.cache_limiter php.ini
setting in my server:
it was set to:
session.cache_limiter = private
This allows browsers (but not proxies) to cache page contents.
Of 2 servers only one was "misconfigured", honestly I don't remember what
the default is

I'm not sure I completely understand your problem. I'm a user of SM and I
don't have a problem with IE 6. I think the Refresh link in the left frame
of SM is only supposed to refresh the left frame. You may want to have a
look at the Folder Synch plugin, maybe this will settle the issues you are
