Someone on the OpenLDAP mailing list was able to come up with the
answer. I can't believe this is not documented someplace in Squirrelmail
but, adding "allow bind_v2" in my slapd.conf file fixed my problem.
I know that OpenLDAP 2.1.22 has v3 only by default. It appears that

I believe so. Outlook on a windows98 machine looks up the info on this
LDAP serber and does not need to supply any credentials.
Is there another line that needs to be added to slapd.conf maybe for
Squirrelmail to work?

I should have read further into the attachment size FAQ I found on SM.
Apache 2.0 has another place, a php.ini file where this needs to be
modified and changing it to the value I wanted, allowd the attachment of
a larger file than before and removed the error page
