I had perhaps the same problem a couple of days ago. I fianlly figured out
that 2 things were happening unexpectedly.
First, the vacation program defaults to only respond to the same address once
a week. So unless you turn off vacation for a user and then turn it back on,
it will not send a rep

The undelete button doesn't always show up when it's supposed to.
Message List:
If "trash_folder=none" is in the users prefs, the undelete button
will not show up.

I'm using SquirrelMail on a Windows 2003 Server with Apache. I tried setting
up a alias for the SquirrelMail base URL, in order to provide my users with
a URL they can keep in mind.
I therefore added a alias in Apaches httpd.conf:
