After a few days of frustration I finally figured out the issue. You have to have SSL turned. To create a certificate to test with you can follow the instructions at:
http://developer.apple.com/internet/macosx/modssl.html
Since you are using 10.3 Server ignore all the Apache stuff at the bottom.

I am running on Knoppix (linux) Apache 1.3 w/ eXtremail as my IMAP server.
When I try to login I am getting the following error:
