It appears virtusertable plugin will allow me to accomplish #1 below, and
perhaps virtual_host plugin may achieve #3 if local domain name is used to
login. Is there a way I can achieve #2 along with #3? Or, do I have to tweak
those two plugins to make it work for my application

Minimum requirements for SquirrelMail 1.4 is PHP 4.0.6. YOu might want
to try upgrading to the latest 1.4 cvs version, you may have some
luck.
