Hello,
I'm using PHP/ISAPI 4.3.0 on Win2000 with Squirrel 1.2.11 (using
Mercury as the IMAP module).
Lately I've been getting a lot of errors in the Windows Event
Application Log, specifically
c-client: Undefined index 4 in page_header.php on line 208 and a
bunch of Undefined variable errors.

Please describe each individual problem. We need more details in order
to help solve your problem(s). It is always helpful if you turn on
display_errors in php.ini as well as set your error level to E_ALL.
MySQL is only relevant if the plugin says it is required.

has at least one plugin that might help you migrate your address book.
Other than that, you might have to do it manually...?

You may have already seen this, but this is all I have. This is the case on

Does anyone have idea on how to fix the apache setup so I do not have to put
a slash on the end of the URL

No huge requirements for those two
directories (beside read permissions for all). You should be more concerned
about permissions/ownership for the data and configuration files/directories.
Please consult the squirrelmail website for plenty of tips and how-tos.

It's broken if you use register_globals set to On in the php.ini
file.

For some reason, I can't seem to get this to work. AFAIK, I've
installed it correctly. Is there a "debug" mode that I can use to
figure out what's going on?
