Maybe you should add a line
options smc-ultra io= irq=
in your modules.conf? Not sure if you need both.

Im running RH 6.2 ...for some reason, on bootup eth0 won't load but after
bootup I can run modprobe smc-ultra
ifup eth0
and it brings it up,..no problem and in my modules.conf file I have the
correct lines
alias eth0 smc-ultra
so i added the lines
/sbin/modprobe smc-ultra
ifup eth0
to my /etc/r
