If you're running a kernel before 2.4.19 try upgrading your kernel. I
ran into some issues with pre 2.4.19 kernels that would do some strange
things, apparantly someone did some major nfs work in 2.4.19.

Well, you have a couple of problems:
You don't want to trust eth0, by trustung eth0 ( your only network card
) you are basically disabling the firewall from that interface.

/boot partition on (software) raid5 as far as I know.
What drive was it that failed, your boot drive? Did you install
lilo/grub onto multiple disks?
Any more information about your config could be helpful.
