It looks like my problem is different, since I'm not seeing this:
qemu-img: error while writing sector 65: Input/output error
But maybe, it is a light in the end of the tunnel...

I have tried, many times, to use qemu-img to create StreamOptimized for
VMWare ESXi 6.0 OVAs.
It does NOT work.
After days of research, I replaced qemu-img, by vboxmanage, then, it
worked!
Now, I'm using something this:

vboxmanage convertfromraw packer/output-vm.raw packer/output-vm-disk1.vm

Conclusion:
A QEmu guest router does not route tagged VLAN packages that are
originated at its host, neighter from others guests hosted at the same
hypervisor. Making it impossible to create a virtual network within a
hypervisor.

Guys!
I can confirm that, after disabling KSM, all "guest problems"
disappeared!! All Windows 2008 R2 guests are now very stable under QEmu
2.0 but, KSM is disabled.
* Windows 2008 R2 guests running for about 6 hours without any crash *
Workaround - disabling KSM:

root@hyper-kvm-1:~# cat
